Southwestern Health Resources (SWHR), a Texas-based clinically-integrated network (CIN) of nearly 5,000 physicians, collaborated with Cerner HealtheReferrals to proactively keep patient referrals within its CIN and standardize referral workflows. Within six months of implementing HealtheReferrals, the results have showcased improved retention. As a whole, the organization increased the number of patients who stayed in-network by approximately 29%, bringing in-network referral rates up to nearly 70%. In addition to this increase, the Centers for Medicare and Medicaid Services (CMS) recently announced SWHR as the top-performing Next Generation Accountable Care Organization (ACO) for the third consecutive year, generating $52 million in earned shared savings in 2019.
