Empowering Geisinger and Keystone ACO networks through data and clinical transformation

In this client HIT talk, you'll hear Geisinger and Keystone ACO executives share examples of how data strategies and the HealtheIntent platform helped identify and address an opportunity that helped contribute to quality outcomes. Speakers will discuss the journey moving to common, unified data platform to overcome operational challenges, attributing patients to providers and the integrating insights into the provider workflow. When looking at the data for inpatient rehab admissions, Geisinger found it was higher than the national average and realized there was an opportunity to improve. Care design was disjointed and discharge trajectory and treatment patterns were dependent on regional culture and historical patterns rather than what was best for both quality care delivery and most cost effective with resources. Providers were also already focused on quality measures; however, the benchmarks were driven by varied in-house standards versus evaluating against industry best practices.
