Improving the health of patients and communities through Cerner determinants of health

The past year shone a spotlight on health disparities that existed long before COVID-19 and created opportunities to address the underlying causes of these inequities. The new Cerner determinants of health product is a comprehensive offering focused on addressing social risk factors across patients and communities. You'll gain insights into the capabilities of the current product and future enhancements, including screening assessments, automated goals and activities, and an analytics dashboard that provides insights into community- and patient-level vulnerabilities.
